TYPE 1 Opt Out Form

NHS Digital has been collecting data from GPs through its trusted General Practice Extraction Service. This system is now being replaced with their new General Practice Data for Planning and Research (GPDPR) service, a broader general-purpose collection which will enable faster access to pseudonymised patient data for planners and researchers. 

 

The data collection will begin on 1st July.

Opting out

If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out or a National Data Opt-out, or both. These opt-outs are different and they are explained in more detail on the NHS digital website. Your individual care will not be affected if you opt-out using either option.

Telephone Triage

We now offer telephone triage.  Triage just means deciding what, if any, treatment you need and which clinician you need to be seen by to get this treatment.  Offering telephone triage means that you will be able to speak to a clinician on the same day and either your problem dealt with over the phone, if appropriate, or an appointment offered if the clinician feels you need to be seen.  We hope this will work well and help improve access to appointments. 

Prescriptions
During Covid 19 you may order your prescriptions by phone at any time during the day not just from 2pm til 5pm.  You can also order on line.   Please allow 2 working days before collection and make allowances for weekends and public holidays.

Coronavirus

The NHS in Durham Dales CCG and Public Health England (PHE) are extremely well prepared for outbreaks of new infectious diseases. The NHS has put in place measures to ensure the safety of all patients and NHS staff.

From 01/04/2020 ABL Health will be delivering the Stop Smoking Service across County Durham. Our person-centred service focuses on delivering flexible specialist stop smoking support to those who live or work in County Durham.

Throughout the Coronavirus outbreak our Smokefree County Durham service is still open.

Covid Restriction Update 

Throughout the pandemic we have strived to make the surgery as safe as possible for all our patients and staff. We will continue to do so going forward.

From 19th July nothing will change at the surgery – patients will need to still wear a mask, socially distance and use hand sanitiser in the surgery and staff will continue to wear PPE.  We do understand that it can be frustrating but we know when people come to us they are sick and vulnerable so we feel it would be wrong not to take these small precautions to protect people. 

We also need to protect our clinicians – we could lose hundreds of appointments each week if our clinicans become covid positive.

 

In terms of opening up, as many of you know, we have never been shut.  In fact we have consulted with much higher numbers of patients than we did pre covid – face to face, telephone and video.  We have also been a part of the vaccination programme.

 Please help us minimise the impact of covid , keep our patients and staff safe.

If your problem is more urgent please contact the surgery and a telephone triage appointment or a F2F appointment will be arranged.

Welcome to Auckland Medical Group

Auckland Medical Group is one of the largest general practices in County Durham practicing from three branches based at Watling Road, St Helens and Toft Hill.

We are a teaching and training practice. We currently have in excess of 14,800 patients registered with 7 General Practitioners.  We have 4 nurse practitioners, 5 practice nurses and 5 nursing assistants.

This is an enthusiastic, motivated and patient orientated practice always striving to improve services for our patients. 

We provide a wide range of general medical services including immunisation, sexual health services,  physiotherapy and minor surgical services including cryotherapy, chiropody, smoking cessation, weight reduction, asthma, COPD, heart disease, diabetic, mental health and health checks. We are also a Yellow Fever Centre and our nurses can offer travel advice and vaccinations.

We also dispense to 20% of the practice population who live in rural location such as Hamsterley, Witton le Wear, Etherley and Toft Hill.

All sites have been adapted to meet the needs of patients with disabilities. Please speak to the reception staff if you have any special requirements. Consulting rooms are all located on the ground floor with ramp access to all surgeries

Communication

We would like to get better at communicating with our patients.  We want to make sure you can read and understand the information we send you.  If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.
